There is no shortage of advice about how to train for a longer, healthier life. None of us really has time for all that anyway. Unless, of course, it’s a 3-hour optimisation sermon, delivered by a podcaster confidently freelancing well past their field.

So, it’s worth paying attention when a large, decades-long study lands and answers a question many of us have: how much strength training do you need to live longer? Or rather, how little strength training do you need to keep the benefits later in life?

A team led by researchers at the Harvard T.H. Chan School of Public Health tracked 147,374 adults for up to 30 years and published the results in the British Journal of Sports Medicine in 2026. They found that the amount of lifting linked to the biggest reduction in death risk is smaller than most people assume, and pairing it with regular cardio does more for you than additional strength training.

Let’s dive in.

What the Researchers Wanted to Know

The benefits of aerobic exercise for living longer are already well established. Walking, cycling, swimming and similar activities have a long track record in the research. Strength training has been less clear. For context, the 2018 US Physical Activity Guidelines recommend 150 to 300 minutes of moderate-intensity or 75 to 150 minutes of vigorous-intensity aerobic activity each week, plus muscle-strengthening activity on two or more days. That muscle-strengthening advice was based on limited evidence, and most earlier studies compared people who lifted two or more times a week against those who did less, without pinning down how much was ideal.

The Harvard team had three questions:

  • Is strength training linked to a lower risk of dying, both overall and from heart disease, cancer and neurological disease?

  • Is there a point where doing more strength training stops adding benefit?

  • How do strength training and cardio work together when someone does both?

Methods

The researchers pooled three long-running US studies: the Health Professionals Follow-up Study, which follows male health professionals, and the Nurses' Health Study and Nurses' Health Study II, which follow female nurses. Together, these gave 147,374 participants, 31,540 men and 115,834 women, with a median age of 54 at the start. Over the follow-up period, which ran up to 30 years, 35,798 participants died.

Participants reported how much time they spent each week on strength training and on aerobic exercise. The important detail is that they were asked again every two years rather than only once. The researchers then averaged each person's repeated answers to represent their long-term habit.

Most earlier studies measured exercise a single time and assumed it stayed constant for decades, which introduces error. Measuring it repeatedly gives a more accurate picture of what people actually did over many years, and it is one of the main reasons this study carries more weight than a typical exercise survey.

A few other design choices strengthen the findings. People already diagnosed with cancer, heart disease or stroke at the start were excluded, and the researchers built in a two-year gap between measuring exercise and counting deaths. Both steps reduce the chance that existing illness, rather than exercise habits, drove the results. The analysis also adjusted for age, smoking, diet quality, alcohol, family history, body weight and aerobic activity, so the strength training results reflect what lifting contributes independently of cardio.

What They Found

The sweet spot for living longer

Compared with people who did no strength training, the lowest risk of death from any cause showed up at around 90 to 120 minutes per week. People training 90 to 119 minutes had about a 13% lower risk of dying during the study (hazard ratio 0.87).

The benefit grew as people did more, up to that point, and then flattened. Those doing 120 minutes or more per week still had a lower risk than non-lifters, about 8% lower, but they did no better than the people training 90 to 120 minutes. So, there was no extra longevity payoff for higher training volumes.

Heart disease

For deaths from cardiovascular disease, that same 90 to 119 minutes per week was linked to a 19% lower risk (hazard ratio 0.81). This was one of the larger effects in the study, and it fits with what is already known about strength training and heart health.

Brain and neurological disease

The largest single-cause result was for neurological disease, most of which was dementia in this data. At 90 to 119 minutes per week, the risk of dying from a neurological disease was 27% lower (hazard ratio 0.73). Trials that use brain imaging have suggested that strength training can produce structural changes in the ageing brain, which offers a plausible reason for the link. The researchers are careful about how far to push this one, and their caution is covered in the limitations below.

Cancer, the exception

Cancer did not follow the same pattern as everything else. A lower risk of cancer death appeared only at lower amounts of strength training, roughly 1 to 59 minutes per week (hazard ratios of 0.91 for 1 to 29 minutes and 0.88 for 30 to 59 minutes). At higher volumes, the association disappeared.

The effect was strongest for cancers already tied to physical activity, and it was driven mainly by lower deaths from colorectal, bladder and breast cancer, although the number of cases here was small. The researchers suggest a possible explanation involving IGF-1, a growth factor that can rise with heavier training and has been linked to some cancers, but they are clear this is a hypothesis rather than a settled finding. The everyday takeaway is modest: a small amount of strength training was associated with lower cancer death, and there was no sign that more was better for this particular outcome.

Strength plus cardio, the combination that worked best

When the researchers looked at both types of exercise together, a clear order emerged. Doing strength training on its own, with little or no cardio, was linked to a 7 to 11% lower risk of death. Doing cardio on its own carried a larger benefit, in the range of 26 to 43%, depending on the amount. The lowest risk of all belonged to people who did high levels of both.

There was also a ceiling. Once someone was doing a lot of cardio (around 45 MET-hours per week or more, which is several times the minimum in the physical activity guidelines), adding strength training on top did not lower their risk any further. Below that level, strength training kept adding value at every amount of cardio. For most people who are not endurance athletes, that means strength work and cardio each earn their place.

Practical Takeaway

For overall longevity, plus the heart and brain benefits, the amount of strength training linked to the lowest risk sits at roughly 90 to 120 minutes a week. That’s is about two solid sessions, and doing more than that did not improve the numbers in this study.

Cardio still did more of the work for reducing death risk overall, and the best results came from combining the two. A sensible approach for most people is to keep two strength sessions a week as a consistent habit and pair them with regular cardio they will actually keep doing, whether that is walking, cycling, swimming or anything else that gets them moving.

None of this means the training should be aimless. Structured, progressive strength work will get you stronger and deliver benefits this study did not measure, such as better physical function, balance and independence as you age. The 90 to 120 minute figure describes the longevity signal specifically. It is not a limit on everything worth doing in a gym.

The other message worth taking from this is about starting. The amount of lifting associated with real benefit is modest and achievable. If you have been delaying because you think you need to train far more, or because the whole thing feels daunting, the evidence points to two structured sessions a week being a genuinely worthwhile starting point. Building the habit earlier protects more of the strength and independence you will want to keep in later life.

What the Study Can and Can’t Tell Us

This is an observational study, so it can show a strong and consistent link between strength training and lower death risk, but it can’t prove that the training itself is the cause. People who lift regularly tend to differ in other ways, and while the researchers adjusted for many of those factors, they can’t rule out every one of them.

Exercise was self-reported, and the questionnaire recorded time per week rather than intensity, load or number of sets. The study, therefore, cannot say how heavy or how hard people trained, only roughly how long they spent. The authors note that the health effects of strength training may well differ by intensity, and that this is a question for future research.

The cancer finding rests on smaller numbers and is best treated as a signal worth investigating rather than a firm conclusion. The neurological finding carries its own caution, because dementia develops over many years and can reduce someone's activity long before it is diagnosed, which can blur cause and effect. The association did hold up even when the researchers built in longer gaps of eight to twelve years, which is reassuring, but they still advise reading it carefully.

Finally, the participants were mostly white, middle-aged to older health professionals. The authors point out that the benefits of meeting activity guidelines tend to hold across different ethnic groups, but the specific numbers here may not transfer perfectly to every population.

More than that is fine if you want it for strength, aesthetics, sport or enjoyment, but this data showed it didn’t buy extra years.
